Open the opt-out page and verify that the form asks for first name, last name, and state.

Start with the state associated with the current listing.
Enter the first and last name shown on the record, select the state, and choose Search.

Step 3
Use locations and age rather than name alone.

The site displays a green arrow or continuation control for the selected result.
